Categories are 0-6 month, 7-12 month and 13-18 months.
PRE-REGISTRATION is required!
Registration fee is $10 until August 7th, then $15 until August 13th, and no registrations will be accepted after August 13th. Registration forms can be picked up at: Columbus Chamber of Commerce office, 753 33rd Ave., Shelley Stempek's State Farm Office, 2457 33rd Ave., The Corner Stop, 220 23rd Street or at the CASA Connection office located at 1465 27th Ave. in Columbus. We can also email or fax one to you.
